‘I Couldn’t Even Open My Eyes. That’s The Extent of the Swelling I Was’

Onemanda Preisinger feels anxious about her child’s impending 13th birthday party. Not for the usual causes related to a house full of clamorous preteen kids, but since it’s the first time she will debut her recently altered face to friends and extended family. “Clearly I’m going to inform everyone as they arrive, ‘Just so you know, this is not how I appear,’” states the 30-year-old real estate agent from Southern Florida.

How she looks is, well, a somewhat surprising – her face swollen and unnaturally tightened, as though held together by heavy-duty tape. Her new – and she’s eager to emphasize, short-term – appearance is the outcome of half a dozen aesthetic surgeries, such as an minimally invasive facelift, conducted by a doctor in Turkey’s Istanbul, the previous month. “My poor husband became emotional when he saw me for the initial time because I couldn’t even open my eyes. That’s how puffy I was,” she explains to me via video call from her house. “I needed to inform him: ‘Babe, I’m fine, I’m not hurting. I just look like someone jumped me.’”

Increasing Popularity of Early Facelifts

According to plastic surgeons, Preisinger is among a growing number of people choosing to undergo a rhytidectomy in their twenties and thirties – well over a decade prior to most doctors’ usual candidate age of 40 to 60. (Though most surgeons are eager to highlight the industry edict of: “We address genetics, not age.”) “I have 28-year-olds requesting facelifts,” says based in London aesthetic plastic surgeon Georgios Orfaniotis, a former NHS consultant who focuses on the facial area. “It’s a very significant surgery, and I’m a bit concerned when I observe individuals opting for it as a lifestyle choice.”

Explaining the Facelift Procedure

A facelift, alternatively called a rhytidectomy, elevates the tissues in the face that begin to droop as we grow older. “Human faces are structured a little like onion layers,” explains Kent-based face specialist Marc Pacifico. “Dermis and adipose on the top, then a stratum of connective tissue called the superficial musculoaponeurotic system. Consider the facial framework as the structural foundation of the face. And that is what causes aging; that is what becomes lax and pulls the dermis downward with it.”

Previously reserved of moneyed older people, overuse of injectables – when excessive use of injectable gels expands the face and causes laxity in the dermis – alongside the common adoption of slimming medications, low-cost medical tourism, and the advancement of new, celebrity-endorsed operative methods are attracting a new kind of patient towards this invasive surgery. Statistics show an eight percent rise in facelifts over the past 12 months in the United Kingdom; while a survey revealed that the percentage of younger facelift patients is increasing, with one-third of procedures now conducted for individuals aged thirty-five to fifty-five.

“People are asking now to appear as the optimal form of themselves and obviously the techniques are following,” comments Orfaniotis. At present, the operation du jour is the comprehensive facial lift, a technique promoted by figures including Kardashian matriarch Kris Jenner, to fashion designer Marc Jacobs.

Deep Plane Technique

While a conventional rhytidectomy entails elevating the SMAS, the comprehensive lift loosens the facial ligaments beneath it, allowing doctors to reshape the face by moving the deeper tissue, and avoiding the taut, pulled appearance sometimes wrought by more traditional techniques. “We avoid placing stress on the dermis because we’re targeting deeper, to the deeper tissue,” explains Harley Street plastic surgeon Rajiv Grover, who specialises in the advanced method. “It is the underlying layers that is adjusted. The dermis follows along as a secondary element.” Lifting the whole facial structure as a unified block results in a authentic-appearing and longer-lasting result. It also means that the surgery is not just being used for the primary goal of anti-ageing – or revitalization, as the aesthetics industry describes it – but for “enhancement” and contouring, too. “We’re getting many inquiries from patients aged late twenties to mid-thirties for this purpose,” says Grover.

Personal Journey

Preisinger had not intended on undergoing a facelift – at least not at the age of 30. But years of using hyaluronic acid dermal fillers in an effort at “facial balancing” implied that by the time she reached her late 20s, she was “botched”. “I didn’t do this because I wanted to look young,” she says. “I underwent it because the injectable ruined my face. I wish I had never using it. If I had avoided the injectable, I don’t believe I would be where I’m at currently.”

If dermal fillers completely disappear has historically been a debated topic in the cosmetic field. “Research have indicated that they seldom completely dissolve,” states Grover, “but they migrate and can obstruct drainage pathways. One of the contributors to what we term ‘puffy appearance’ is also the fact that to some extent, the face becomes slightly waterlogged because its ability to drain bodily fluid has been reduced.” Although studies into the topic is early-stage, cautionary statements on dermal fillers’ possible effects on the body’s drainage have been issued, and further research initiated. An esteemed face specialist, speaking on the condition of anonymity, mentioned he would avoid using injectables in a client because of the dangers they pose. “Many doctors avoid talk about this, because the manufacturers who make injectables can be quite forceful.” He’s been told accounts, he says, of plastic surgeons facing court orders after expressing worries.

Choice and Surgery

For Preisinger, after beginning injecting filler, she believed she had to keep replenishing it – particularly when it began moving to other parts of her face. She was only twenty-eight when a nearby cosmetic doctor in the state suggested that a facial and neck surgery could be what she required to exit the cycle of treatments. After 24 months, she ended up choosing between a list of suggested accommodations in the city, texted to her by the patient coordinator at her surgeon’s office.

Initially, Preisinger had scheduled an eyelid surgery – better known as blepharoplasty – but her doctor advised her that a facelift was the correct option for the laxity she was experiencing in her face. She arranged a brow lift, a mid-facelift and a neck lift. The day before her operation, 24 hours post she’d touched down alone in Turkey, she opted to include a mouth enhancement. “Subsequently the surgeon stated, ‘We will elevate the lower eyelid puffiness.’ He added, ‘I believe if we remove some buccal fat, it will sculpt your face.’ Thus I ultimately adding two more surgeries,” she recalls. She spent $22,000 (sixteen thousand five hundred pounds) for the half-day operation, including a three-day|
